Direct Primary Care is a model which operates outside of the typical insurance system. Rather than contracting with insurance, paying co-pays, and asking for insurance authorizations, Direct Primary Care works on a direct connection between the patient and the practice. Patients agree to pay a monthly membership fee, and in return receive all their primary care services under one roof without additional costs for visits during the year. This means that folks who do not have insurance can receive care for much less than they would pay for Emergency Department or repeated urgent care visit, and have access to discounted lab tests and in-office tests. Folks who do have insurance but who value this model of care and the individualized attention we can offer can pay the monthly fee, and tests, medicines and other services can be covered by medical insurance.
Direct Primary Care allows members to be able to predict their healthcare costs and to partner with us to keep out-of-pocket costs low. It also allows for increased privacy for DPC patients, since there is no outside billing needed.
There may be small costs for in-office testing (strep test, urine testing, etc.) and for medications dispensed at the office. Lab tests can be billed to patients' insurance, if that is what folks prefer, or will be charged at LabCorp's cash pay rates. These lab costs would be paid at the time of the lab collection, and are usually much, much, much less than folks would be charged through their insurance.
Since Direct Primary Care practices are supported by their members, we do not depend on co-pays with each visit to keep the doors open. This means that we can offer much better access than traditional practices. We can spend more time with each visit--30 or even 60 minute visits are standard. We will also keep slots available every day for same-day and urgent care visits so that you will not need to wait for care or spend extra money. We can also meet you where you need us: in person, over video virtual visits, over the telephone, or even by text or secure portal messages. This model gives you the flexibility you may need to take good care of yourself!
Direct Primary Care is not health insurance, and it will not cover any subspecialty care, emergency care, surgeries or hospitalizations. We encourage all patients to consider catastrophic insurance coverage for these unexpected (and expensive!) medical costs.
Gender Affirming Care
We believe that every person should be given the opportunity to live life as their true and authentic selves, and that medical practices and professionals are in a position to help folks through their journey. Through their own journey towards understanding their gender and starting gender-affirming care, Mark was fortunate to work with affirming and caring professionals, and hopes to provide the same affirming care for folks who trust their care to Sanctuary Family Medicine.
The SFM team will meet patients where they are in their journey and will help them through determining their next steps, including medication management and hormone treatment. Sanctuary Family Medicine will be a safe and affirming space for everyone, and will also be able to make a connection to other affirming providers, including counselors and surgeons, when patients need this additional care.
Pediatric Gender Affirming Care
We know that federal policy decisions have greatly limited access for patients and families seeking gender affirming care for patients under 19 years old. This is a disastrous and awful decision that has already hurt many Virginians and will hurt more in the future. Because Sanctuary Family Medicine operates outside of the insurance system, we are not affected by these federal and local health system decisions: we can still provide gender affirming care for young people. Queer and Sexuality Affirming Care
Our vision at Sanctuary Family Medicine is that all folks who seek care with us will find a welcoming and affirming space that values them and their health, in all areas. We will be a medical practice where patients can ask any questions, share any information, and ask for any help to meet their health goals. We will provide pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis, Doxy-PrEP, and testing and treatment for sexually-transmitted infections, when needed. We are excited to work with folks in traditional relationships, non-traditional relationships, polyamorous relationships, and kink practices.
Sanctuary Family Medicine at this time will offer certain types of contraception, including Depo shots and oral contraceptive medications (birth control pills). In the future, we plan to offer Nexplanon implantable birth control. For patients who need higher levels of care, such as intrauterine devices (IUDs) and abortion care, until we are able to offer these services we will help connect patients to community resources and providers who can meet these needs.
Virginia allows physicians' offices to dispense medications to their patients. Sanctuary Family Medicine is excited to offer this additional benefit to our patients. We plan to offer medicines for acute illness, like antibiotics, with the goal to expand our options as we move forward. We plan to provide gender affirming hormones, especially for patients who may have difficulties finding these medicines in a more traditional medical practice. These medicines will be made available at cost, based on what we pay for the medications themselves as well as the labels, medicine bottles, etc.
Dr. Ryan is able to provide many standard behavioral health interventions and medication management for psychiatric conditions as part of their family medicine training. For folks who need care beyond what Sanctuary Family Medicine can offer themselves, they will make connections to trusted colleagues. In the future we hope to include specialized behavioral health services directly in our own office.
We see Sanctuary Family Medicine as a partnership between you and us. If you are in financial need, we plan to offer reduced pricing per month, which we are estimating to be $65/month per person. We will need to determine how many reduced cost memberships we can offer as we get going. If you are in a more stable financial situation and can afford to pay more than $85/month, we invite you to consider doing so--every member who can pay above the standard monthly fee helps support those who need assistance. This is a nice way to identify where you might find yourself: in this model, Tier 1 would include folks who may need assistance and Tier 3 would include folks who may be able to support others.
Each new member for the practice will need to pay a one-time $100 application and enrollment fee.
